Reporting
All invoices submitted must be accompanied by a report (either on the report form supplied or in your own format). The report must contain the following information:
- Name of Provider
- Name of Patient
- DOB
- Date of Visit
- Service Description
- Site of Injury/Type of Consult
- Notes
- Outcome
- Action/Referral
Invoices must be submitted to the NZ Academy of Sport : South Island, PO Box 6087, Dunedin by 8TH OF THE MONTH FOLLOWING to ensure payment on or about 20th of the month following.
How to become a Provider
Below are the criteria to become a Preferred Provider. For further information please contact

Physiotherapy
Providers must meet all or most of the following criteria:
- 1. New Zealand Registered Physiotherapist
- 2. Hold a postgraduate qualification in Sports/Manipulative Physiotherapy or a related field
- 3. Qualified and have been practicing for more than 5 years
- 4. Involvement with regional, national, or international sports teams and/or elite athletes
- 5. Member of the New Zealand Society of Physiotherapist
- 6. Member of Sports Medicine New Zealand
- 7. Assoc. member/Member of New Zealand College of Physiotherapy
- 8. Member of the New Zealand Sports & Orthopaedic Physiotherapy Association
- 9. Membership of the Physiotherapy Practice Accreditation Scheme

Medical
These criteria are as follows:
- 1. A recognised postgraduate qualification in sports medicine
- 2. Active membership of Sports Medicine NZ
- 3. Qualified and have been practicing for more than 5 years
- 4. Involvement with regional, national, or international sports teams and/or elite athletes
- 5.) Good standing with the Medical Council of NZ
- 6. Evidence of clinical involvement with teams or individual athletes.
- 7. Willingness to participate in academic or collegial activities including relevant conferences or Academy seminars.
Science
The New Zealand Academy of Sport in association with Sports Science New Zealand has set levels of accreditation in sport science disciplines. Generally the minimum level, to consult with athletes in the Academy is Level 2.
